Physical Therapy
Hearts With Integrity’s physical therapy program aims to maintain and restore strength, increase endurance and range of motion, relieve pain and improve functional capabilities. Physical Therapy services include: Gait training, Therapeutic exercise and Transfer techniques and recommendation and training in assistive device use.
Hearts with integrity’s Physical Therapy program is ready to meet the home rehabilitation needs of joint-replacement surgery patients.
Hearts With Integrity provides physical therapy as needed for Fall prevention. Our team of friendly and professional Physical Therapists evaluate and develops individual therapeutic plans based on your physician orders for patients with mobility problems and functional limitations.
WILL MY INSURANCE PAY FOR PHYSICAL THERAPY?
Patients qualify for Medicare-covered home health services if the following conditions are met:
Patient is homebound or confined to home due to illness, injury, or post-operation. Services are provided under a plan of care established and approved by a physician. Services are medically necessary and reasonable. Services are performed in an individual’s place of residence. Patient requires skilled nursing or physical therapy on an intermittent basis.

Our Social worker uses community resources, long term planning, crisis intervention, and short term therapy to help patients resolve issues. Social worker collaborates with doctors, nurses, therapists, caregivers, and patient to assure that the patient’s needs are met in a comfortable and safe environment.
Insurance Coverage of Social Worker
Medicare: Reimburses for social workers to visit patients at no cost to patient Medicaid and Private Insurance: These insurance do not reimburse, but patients will be served if Social Work needs arise.
Steps for getting Social Services for your patient:
1. Patient must be homebound and receiving skilled nursing or therapy.
2.Then, include MSW evaluation in discharge orders or Physicians can add MSW to patient’s home health plan of care or Call with verbal orders for MSW
3.Social worker completes an assessment and completes any follow-up necessary for patient. Also, Social Worker can follow up with person who made referral to assure continuity of care.

Occupational Therapy
Our Occupational Therapy Services allow for continued medical treatment in the comfort and familiar environment of our patient’s home, their family’s home or an Assisted Living facility. We will assess safety conditions in your home and evaluate for needs of adaptive equipment. We work with you, your physician and your family to develop a plan of care that will meet your rehabilitative needs. Our goal is to improve fine motor coordination and prepare our patients to resume day-to-day activities by educating the patient about joint protection and energy conservation.
